Social Worker II

Location: Central & Western District

Roles & responsibilities:

  • Improve the social determinants of health, including support the engagement of the Central & Western District Health Centre (C&W DHC) members and their family and the implementation of medical-social collaboration and community engagement under the C&W DHC services;
  • identify psychosocial needs of clients and provide counselling services;
  • execute patient empowerment programs, community health programs in C&W DHC and community outreaching services spots;
  • provide support to community education activities, community health assessment services and home visits organized by the C&W DHC;
  • work with multi-disciplinary professional teams to deliver primary health care services; liaise with different stakeholder on community resources mobilization and service collaboration; and
  • perform any other duties as assigned.

The ideal candidate should have/be:

  • a recognized Higher Diploma in Social Work or equivalent;
  • a registration under the Social Workers Registration Ordinance (Cap. 505) as Registered Social Worker;
  • at least Grade E or above in English Language (Syllabus B) and Chinese Language in the HKCEE or equivalent;
  • preferably 5 years of post-qualification in social or medical service sector or equivalent; and
  • good computer literacy.

Compensation & employment terms:

The successful candidate will be appointed on contract terms with a salary currently at $31,795 per month (Tung Wah Master Pay Scale Point 13) plus a allowance currently at $1,000.  Entry salary depends on years of recognized experience.  The salary is subject to downward/upward adjustment or may be frozen as determined by Tung Wah and/or as appropriate with reference to the civil service pay review.  The fringe benefits include paid annual leave, medical services, a dental scheme and training sponsorships.
